How to Apply for Medicaid for pregnant women
Listed below are the locations and methods in which you can apply for pregnancy (SOBRA) Medicaid. Please review the page "MomCare Eligibility" before determining which method is best suited for you.
There are three (3) ways to apply for Medicaid for pregnant women:
- Presumptively Eligible Pregnant Women (PEPW): Please visit one of the locations listed below for pregnancy testing, prenatal care, and also to complete the PEPW application. Please note that if you apply for Medicaid coverage at one of these sites, this is where you will continue to receive your prenatal care.

- Simplified Eligibility for Pregnant Women (SEPW): A simplified application “full coverage” Medicaid for pregnant women only. The application can be faxed and mailed to the Department of Children and Families (DCF) number and address listed below. You can also apply for up to three months of retroactive Medicaid coverage if you were pregnant in those prior months and had medical services rendered. Call the MomCare Program to obtain an SEPW application or print it from the page "MomCare Eligibility".

3. Access application: This is an application for "regular" Medicaid, including children, cash assistance and/or food stamps. Click this link to apply online. http://www.myflorida.com/accessflorida/.

If you are approved for pregnancy (SOBRA) Medicaid, you will be contacted by a MomCare representative, known as a Maternity Care Advisor. If you do not hear from a Maternity Care Advisor within 3 weeks of your Medicaid application, please call the MomCare Program at (954) 567-7174, extension 319 or 317.
